Appointments
Procedures and Consent Forms for Wellness Sessions, Reiki or Integrative Relaxation Sessions vary slightly. Please be sure to follow the procedures for appropriate service.
Procedure for Wellness Sessions:
- Please read the Medical Disclaimer. By purchasing any service, you are agreeing to be bound by this disclaimer.
- Contact me to set up a time for your free 30 minute phone session. During that session , I learn what issue or area you need help with. I answer any questions and tell you more about what I do. If time allows, we have a mini coaching session.
- If you choose to purchase a session(s) you must complete two forms which can be accessed as pdf files on this site. These forms are; Client Questionnaire and Informed Consent. These forms can also be sent via email attachment, ground mail or fax.
- You return forms to me with personal check made to Buddy Ross 636 Koontz Rd. Wadsworth OH 44281 or select paypal option on website.
- Once forms and payment is received, I send you an email with instructions on how to take the 12 dimensional Wellness Inventory. You then have an on-line subscription to the Wellness Inventory for 1 year.
- We set an appointment time for either in-person or phone wellness session.
The above steps are completed prior to our 1st phone or in-person session.

- Pre-payment can be made via paypal, personal check mailed to above address and received prior to treatment, or cash only at time of service.

Payment terms
Holistic Wellness Buddy LLC uses PayPal as a secure payment gateway for online payments. At order checkout, you will be redirected to Paypal's secure site where your payment will be processed. We accept Visa, Mastercard, American Express and Discover. Paypal also provides you with the added convenience of being able to pay using an electronic check or funds from your paypal account. If you already have a Paypal account, you will be given the opportunity to login. Please note: You may be given the option to create a Paypal account during the checkout process. Creating an account is completely optional and not required for you to make a purchase from us using your credit card.
